How does Form to PDF AI work?
Set up once in Google Sheets or Google Forms. After that, every response becomes a filled PDF on its own.
Upload your PDF form
Open the add-on in Google Sheets or Google Forms and upload the PDF you already hand out. AI finds every field, including printed boxes, lines and tick boxes on flat PDFs.
Review and create the Google Form
Rename, retype, reorder or remove fields, mark required questions, then create the Google Form in one click. In Google Forms, the questions are added to the form you have open.
Get a filled PDF for every response
Each new response fills the original PDF and saves it to a Drive folder. In Google Sheets, the PDF link and status also appear on the response row.
Already have responses? In Google Sheets, select the rows and choose Fill selected rows. In Google Forms, choose Fill missing PDFs.
Why keep the original PDF?
Because the people who receive it expect that exact form. Form to PDF AI writes answers onto your existing layout instead of making you rebuild it.
- ✓No rebuilding the form as a Docs template
- ✓No need for fillable fields: flat PDFs work too
- ✓No second account or third-party response database
- ✓Responses stay in your Google Form and Sheet
Which PDF forms work?
Most digital PDF forms, fillable or flat.
- ✓Fillable PDFs: answers go into the form's own fields
- ✓Flat PDFs: text lines, boxes, tick boxes and one-letter-per-box fields
- ✓Choice questions: ticks, or a circle around "circle one" options
- ✓Non-Latin answers, such as Chinese names
Rotated pages and skewed scans are not supported yet. Signatures are typed names.

FAQs
Do respondents need to install anything?
No. Respondents fill a normal Google Form. Only the person who sets up the form installs the add-on.
Can I start from a Google Form I already have?
Yes. Open Form to PDF AI from the add-ons menu in that form and upload the PDF. The PDF's questions are added after your existing ones, and new responses fill the PDF.
Where are the filled PDFs saved?
In a folder in your own Google Drive. When you set up in Google Sheets, each response row also gets the PDF link and a status.
Does it work with PDFs that have no fillable fields?
Yes. AI detects the blanks, boxes and tick boxes on flat PDFs, and you review them before the Google Form is created.
Can I create PDFs for responses I already have?
Yes. In Google Sheets, select the rows in the response Sheet and choose Fill selected rows. In Google Forms, choose Fill missing PDFs in the add-on.
Do you store my PDFs or form answers?
No. PDFs and answers are processed for each request and not stored on our servers. Your files stay in your Google Drive. See the Form to PDF AI privacy policy.
Is my data used for model training?
No. Your PDFs and answers are not used to train AI models.
Where can I get support?
