Form to PDF AI Terms of Service

1. Who we are and acceptance

Form to PDF AI (the "Service") is provided by Simple Metrics Ltd ("SimpleMetrics", "we"). By installing or using the Service, you agree to these Terms.

2. The Service

The Service is an add-on for Google Sheets and Google Forms that detects the fields in a PDF form, creates a Google Form from them, and fills the original PDF with Google Form responses or Sheet rows, saving the results to your Google Drive.

3. Your content and responsibilities

  • You keep all rights to the PDFs you upload, the answers you collect and the filled PDFs.
  • You must have the right to use and fill the PDF forms you upload, and a lawful basis to collect and process your respondents' answers, including telling respondents how their answers are used.
  • AI field detection can miss or misplace fields. Review the detected fields and check filled PDFs before relying on them, especially for legal, medical, tax or financial forms.
  • You must not use the Service for illegal, fraudulent or harmful purposes, to forge documents, or to interfere with or access the Service in unauthorised ways.

4. Plans and usage limits

The Free plan includes 10 filled PDFs per month. The Pro plan costs US$15 per month and includes 500 filled PDFs per month. Setting up new forms is limited per user per day. PDF limits reset each calendar month and unused PDFs do not roll over. We may change plans and prices with notice; changes apply from your next billing period.

5. Billing and cancellation

Pro is billed monthly in advance through Stripe and renews automatically until cancelled. You can cancel at any time from the add-on's billing page; Pro stays active until the end of the paid period.

6. Refunds

If you have not used Pro, we offer a full refund for payments made within the last 30 days. For used subscriptions, refunds are decided case by case. Email [email protected] with your purchase email.

8. Intellectual property

The software, design and branding of the Service belong to SimpleMetrics or its licensors. These Terms do not give you any rights to them except to use the Service.

9. Suspension and termination

We may suspend or end access to the Service if you break these Terms or if needed to protect the Service or other users. You can stop using the Service at any time by uninstalling it.

10. Disclaimer and liability

The Service is provided "as is". To the extent the law allows, SimpleMetrics is not liable for indirect or consequential losses, or for errors in detected fields or filled PDFs, and our total liability is limited to the amount you paid for the Service in the 12 months before the claim. Nothing in these Terms limits liability that cannot be limited by law.

We may update these Terms. We will post changes on this page and update the date above.
